Constant tearing can occur when there is a disruption in the normal drainage of tears. The obstruction can be located at different points along the tear duct and can occur in both children and adults.
An assessment allows the cause to be identified and the most appropriate treatment to be determined.
Why are my eyes constantly tearing?
Tears normally drain through small holes in the eyelids called lacrimal puncta, continue through the canaliculi and the lacrimal sac until they reach the nose.
When any part of this system narrows or becomes blocked, tears can accumulate and overflow onto the cheek. Some patients may also experience discharge, irritation, or recurring infections.
How is the lacrimal system evaluated?
The examination allows us to determine if there is an obstruction and in what part of the tear duct it is located.
In selected cases, it may be necessary to supplement the examination with imaging studies.
Depending on the case, in-office tests such as tear duct irrigation and Jones test can be performed, which help to evaluate the passage of tears to the nose.
Treatment of tear duct disorders
Treatment depends on the location and cause of the obstruction. Not all patients with excessive tearing require surgery, and those who do, do not all require the same type of surgery.
The procedures that can be performed include:

Probing and irrigation: allows exploration and, in certain cases, opening the tear duct.

Punctoplasty: widens a narrowed lacrimal punctum to improve tear drainage.

Silicone tubes: These are temporarily placed inside the tear duct to keep it open during healing.

Dacryocystorhinostomy (DCR): creates a new drainage pathway between the tear sac and the nose when there is a deeper obstruction.
When should you see a specialist?
It is advisable to assess the tear duct when there is persistent tearing, tears running down the cheek, recurrent discharge, inflammation near the inner corner of the eye, or repeated episodes of infection.
Determining where the problem is located is important for choosing the right treatment.
