Entropion and ectropion are eyelid malpositions, primarily affecting the lower eyelid. They can cause irritation, tearing, red eyes, and, in some cases, damage to the ocular surface. Treatment depends on the cause and severity of the eyelid malposition.
What is entropion?
Entropion occurs when the eyelid margin turns inward, causing the eyelashes to constantly rub against the eye.
This contact can cause a foreign body sensation, irritation, tearing, and corneal injury. It is more common in older adults, although it can also result from scarring or other eyelid abnormalities.
What is ectropion?
Ectropion occurs when the eyelid pulls away from the eye and turns outward, leaving its inner surface exposed.
It can cause constant tearing, dryness, irritation, and redness.
The main causes include age-related laxity of the eyelid tissues, scarring, and facial paralysis.
How are they treated?
Treatment depends on the cause and severity. Artificial tears and lubricants can help protect the eye and reduce discomfort, but they do not correct the eyelid's position.
When the eyelid malposition is significant or persistent, surgery is necessary to reposition the eyelid and restore its proper contact with the eye.
Entropion and ectropion surgery
The surgery is usually performed on an outpatient basis under local anesthesia and sedation. Several techniques exist, and the choice depends on the cause, the degree of eyelid laxity, and the individual patient's characteristics.
The goal is to restore proper eyelid positioning, relieve symptoms, and protect the ocular surface.

It is advisable to seek medical evaluation if the eyelid is turned inwards or outwards, the eyelashes rub against the eye, there is persistent tearing, frequent irritation, or difficulty closing the eye properly.
An oculoplastic evaluation allows the cause to be identified and the most appropriate treatment to be determined.
